Sinus Lift Instruments and Osteotomes in Implant Dentistry

When the upper back teeth are missing, the maxillary sinus often expands and the available bone height shrinks — leaving too little bone for a dental implant. Sinus lift instruments and osteotomes are the tools used to address this, either by elevating the sinus membrane to make room for graft material or by condensing and expanding bone for the implant. This overview explains both groups.

Sinus lift instruments

The aim of a sinus lift (sinus augmentation) is to gently lift the Schneiderian membrane — the thin lining of the sinus floor — without perforating it, creating a space that can be filled with bone graft. Specialised sinus lift instruments have carefully angled, blunt working ends designed to separate the membrane from the bony floor and lateral wall. Many are offered as numbered sets, each with a different curvature to reach different parts of the sinus.

Osteotomes and bone condensers

Osteotomes prepare the implant site by compacting and expanding bone rather than cutting it away. Used with light tapping from a surgical mallet, they push bone outward and apically, improving bone density around the implant and, in the crestal approach, gently raising the sinus floor.

  • Concave (cupped) tips: Often used in sinus work to carry and condense graft material and help protect the membrane.
  • Convex tips: Used for controlled bone compaction and ridge expansion.
  • Straight and offset designs: Depth markings help the surgeon control how far the osteotome advances.

Crestal vs lateral approach

In the crestal (transalveolar) approach, osteotomes raise the sinus floor through the implant site itself — suitable when only a few millimetres of lift are needed. In the lateral window approach, a window is made in the lateral wall and sinus lift instruments elevate the membrane for larger augmentations. The instruments above support both techniques.

Supporting instruments

Bone grafting often accompanies these procedures, so bone scrapers, mills and graft carriers are commonly used alongside sinus and osteotome instruments. Surgical mallets provide the controlled force osteotomes require.

Material and care

Surgical stainless steel is standard for strength and repeated autoclaving. Because membrane elevation depends on smooth, undamaged working ends, inspect tips regularly and store instruments so the delicate ends are protected.
