A scalpel handle looks like the simplest instrument on the tray, yet handle pattern and blade choice noticeably change how an incision feels. Here is a quick guide to the handles and blade sizes used in oral surgery.

The standard: No. 3 pattern handles

The flat No. 3-style handle is the everyday choice in dentistry and accepts the small blade series used intraorally. Variants include standard length and longer 14–15 cm versions for posterior access, and millimetred handles with a measuring scale on the shaft — handy for gauging incision length and depth references without a separate ruler.

Blade sizes that matter in oral surgery

• No. 15 — the small curved belly blade; the default for most intraoral incisions and flap outlines
• No. 15C — a finer, narrower 15; favoured for sulcular incisions and periodontal/microsurgical work
• No. 12 / 12D — hooked blades that cut on the inner curve (12D on both edges); excellent around the distal of last molars and in tunnel/sulcular access
• No. 11 — pointed straight blade for stab incisions, classically for incision and drainage

Specialised handle designs

• Orientable / rotating-head handles let you set the blade at an angle to the shaft, keeping the cutting edge correctly oriented in areas where a straight approach is awkward — posterior sulcular incisions become far more comfortable
• Round handles roll smoothly between the fingers for curved incisions, which many surgeons prefer for flap work
• Micro-blade handles take miniature blades for periodontal plastic and other fine soft-tissue procedures
• Push-to-remove / safety handles release the blade with a slider so it never needs to be gripped by hand

Safe blade handling

Blades are single-use and mounting/removal is a classic sharps-injury moment. Mount and remove blades with a needle holder or a safety-removal handle — never fingers — and discard directly into the sharps container. The handle itself is autoclavable stainless steel and lasts for years.
